Currently, when a new Design Document Review form instance is created (or any form for that matter), the user can attach documents both from inside the CDE (Documents) or by uploading from their own computer.

While in some cases this can be acceptable, for most projects a formal review can only be done on documents inside the CDE, and therefore users should only get the option to link documents from inside the system.

Ideally, this limitation should be set by form, as some forms (e.g., Issues) will likely require file uploads.
